Abstract

With 28 000 deaths per year, primary lung cancer is the leading cause of death related to cancer in France. Gold standard curative treatment for non-small cell lung cancer (NSCLC) remains surgical resction. In this article, we’ll review the latest breakthroughs in surgical management of NSCLC. Indications for curative surgical resections have been updated with the 8th classification for NSCLC and the new pathology classification for lung adenocarcinoma. We witness an increase in sublobar resections, especially lung segmentectomies. Each surgical approache at the surgeon’s disposal present its own benefits and disadvantages. Over the years, thoracic surgeons have gone from conventional thoracotomies to muscle-sparing thoracotomies and finally minimally-invasive surgical approaches, represented by video-assisted thoracoscopic surgery (VATS) and robotic-assisted thoracoscopic surgery (RATS). In order to decrease post-operative morbi-mortality, hospital stay and fasten recovery, surgeons include their patients in fast-track protocols.
