Abstract

to present the one of the largest clinical study with radiofrequency ablation (RFCA) of LVOT in children. 16 children (aged 13 to 17) with symptomatic LVOT were studied before and after the RFCA (with transaortic access) by ECG, echocardiography, 24 hour monitoring, surface ECG mapping, and EPS. The catheter position during the applications near the ostium of the coronary arteries (≥1.2см) was controlled by continuous fluoroscopy and coronarography. The earliest ventricular activation was found in the right Valsalve sinus in 2 pts (12,5%) and in the left Valsalve sinus in 14 pts (87,5%). The ectopic ventricular activation was 40-89 ms earlier than those on ECG. The RFCA performed with the temperature of 50-55C. The elimination of ectopic activity was achieved on the first seconds of heating in all cases. The common success rate was 93,8%. There were no arrhythmia recurrences and complications in this group of children during the follow-up (9 ± 3 month). We found LVOS is 27% among children with ventricular arrhythmia. RFCA in Valsalve sinuses is effective and safe procedure in pediatric patients.
