Abstract

Dr. Jeff Horenstein: Today’s case is that of a 27-year-old gentleman brought by helicopter to our hospital from the site of a high-speed motor vehicle collision. At the scene he had a prolonged extrication from the wreckage of his vehicle, and was found to have an altered mental status. He underwent rapid-sequence intubation in the field by paramedics with etomidate and succinylcholine. He received a liter of normal saline and was put on a midazolam drip for sedation by paramedics, who report he was hypertensive, tachycardic, and agitated en route to the hospital.
