Abstract

A 41-year-old woman with a left-sided gallbladder and an anomaly of the intrahepatic portal vein with cholecystolitheiasis underwent successfully a laparoscopic cholecystectomy. The laparoscopic findings demonstrated that the gallbladder was located at the left side of the hepatic round ligament, and the cystic duct was connected to the anterior side of the left hepatic duct. No other abnormal findings were identified in the peritoneal cavity. The left-sided gallbadder without situs intersus totalis is a rare condition, and only four Japanese patients previously underwent a laparoscopic cholecystectomy. A trocar was inserted at the left side of the hepatic round ligament, resulting in the laparoscopic cholecystectomy without difficulty. The preoperative accurate diagnosis of gallbladder position is important for the treatment of a left-sided gallbladder by a laparoscopic cholecystectomy. A trocar should be inserted from the left side of the hepatic round ligament.
