Abstract

We describe a patient who displayed impairment limited to skilled habitual right finger movements due to a traumatic hematoma in the most caudal part of the left superior frontal gyrus. A 44-year-old right-handed man without notable previous disease presented to our emergency room following a traffic accident. No aphasia or apraxia was observed during examinations. He performed intransitive and transitive gestures and sequential movements smoothly upon command. He could also understand easily the meaning of gestures performed by others. Brain MRI on the 7th day after the accident revealed a hematoma, which measured 15.3 × 17.7 × 17.5 mm, in the most caudal part of the left superior frontal gyrus adjacent to the prefrontal sulcus (figure).
