Abstract

Background: Video presented of surgical treatment of patient 59 years old with hepatocellular carcinoma and tumor thrombus in inferior vena cava and right atrium and possibility of multidisciplinary management of such patients. Methods: By ultrasound and multispiral CT of abdomen and thoracic cavity was founded giant tumor by size 25 × 30 cm of left liver lobe extended to segment 8 with tumor thrombus in left and middle hepatic veins, IVC and right atrium. The level of alfa-fetoprotein was 180 mg/l. After multidisciplinary consilium including oncologist, hepatosurgeon, cardiosurgeon, cardiologist, anesthesiologist was decided to perform operation: left hemihepatectomy with partial resection of segment 8 followed by chemotherapy. Results: Left hemihepatectomy with thrombectomy from inferior vena cava and right atrium under parallel normothermic cardio-pulmonary bypass was successfully performed. Patient was discharged from hospital at 6 postoperative days in satisfactory condition for chemotherapy. Conclusion: Using of cardio-pulmonary bypass and multidisciplinary approach allow to perform such surgical procedures safely for patients.
