Abstract

When a fistula is off the midline and more than 50% of its diameter is away from midline, we label the fistula as a “Lateral Fistula.” We have divided the hard palate into two halves: anterior and posterior halves. So Lateral fistula can occur in the anterior half or posterior half of the hard palate; these are labeled as La (Lateral fistula in the anterior half of the hard palate on the left side) and Lb (Lateral fistula in the posterior half of the hard palate on the left side). If the lateral fistula occurs in the soft palate on the left side, we label it as Lc. Similarly, lateral fistula can occur on the right side as Ra, Rb, and Rc. Management is usually by turn in flaps for nasal layer closure and contralateral mucoperiosteal flap for oral layer cover. If the palate is quite short in cases of Rc or Lc, Pharyngeal flap should be added at the same stage.
