In reply We would like to thank Dr Fankhauser for his comments regarding our report of cataract following diode laser treatment for threshold retinopathy of prematurity. Because cataract is a serious vision-threatening complication of laser therapy in premature infants, other treatment modalities certainly bear investigation. The use of transscleral laser retinopexy is attractive in this regard because it avoids the anterior segment irradiation that occurs when laser treatment is delivered through an indirect ophthalmoscopic system. However, transscleral laser retinopexy has not been widely used in the treatment of threshold retinopathy of prematurity and its safety in the premature infant has not been established. Therefore, we would recommend a cautious, prospective comparison of this new treatment modality with cryotherapy and indirect ophthalmoscopic laser treatment.
