Abstract

The second leading cause of death in familial adenomatous polyposis (FAP) patients is duodenal carcinoma. To describe the natural history of duodenal carcinoma in FAP, researchers prospectively examined 368 patients who were identified through the national polyposis registries of Denmark, Finland, Norway, Sweden, and the Netherlands; patients underwent endoscopy at 2-year intervals from 1990 through 2001. Median age at first upper endoscopy was 37; mean follow-up was 7.6 years. Adenomas were visible in 209 patients at baseline. Interestingly, biopsies of tissue from random locations in patients with normal duodenums revealed adenomas in an additional 28, for a total of 65% with duodenal adenomas at baseline.
