Abstract
Laparotomy with splenectomy and biopsy of the liver and the retroperitoneal lymph nodes has been performed on 80 previously untreated patients with Hodgkin's disease. No postoperative deaths occurred, but one death 6 months later may be attributed to the splenectomy. Nodular sclerosis and mixed cellularity were the most frequent histological types, accounting for 41 and 36 % respectively. 52 % of the patients had splenic and 11 % had liver involvement. 15 % had hepatic lesions lacking Reed‐Sternberg and Hodgkin cells, but otherwise consistent with Hodgkin's disease. As a consequence of the laparotomy findings 19 patients had to be given total node irradiation rather than a mantle field and 17 patients chemotherapy instead of radiotherapy. Explorative laparotomy is recommended in apparently localized Hodgkin's disease as it greatly influences the choice of treatment.
