Abstract

A uterine didelphys with unilateral cervical atresia is a rare Mullerian anomaly. Laparoscopic management can be challenging when the patient has dense adhesions from a previous midline laparotomy. We present a case of a 17-year-old nulligravid adolescent who presented with cyclic pelvic pain. Ultrasound and hysterosalpingogram were consistent with a diagnosis of uterine didelphys with an atretic cervix and resulting hematometra from an obstructed hemiuterus. Six months prior, she had had an exploratory midline laparotomy to evaluate recent findings of an 18 cm pelvic mass.
