Abstract

Society is ageing. West Suffolk alone has 16,000 people over 75 years old. While in many respects a triumph, this is also a challenge, given that older people are major users of health and social care services. The need for effective and appropriate services has become a national priority, addressed in specific initiatives such as the National Service Framework for Older People (DH 2001) and Choosing Health (DH 2004). One of the guiding principles of the intermediate care document, Moving Forward (DH 2002), recognises the challenge for health and social services in working together to promote health and active opportunities for older people, helping them to realise their potential. West Suffolk Intermediate Care has established the Keeping Well Centre in Bury St Edmunds to realise these objectives. It provides a proactive educational service that promotes independence and well-being for those over the age of 65.
