Abstract

背景:體外循環(cardiopulmonary bypass,CPB)時會活化白血球,使白血球穿越血管內皮並且傷害其下組織。Junctional adhesion molecules(JAMs)在白血球附著及穿越移動時扮演了重要的角色。在血管內皮細胞的JAM-B可藉由JAM-C的幫助和白血球上的α4β1 integrin作用,參與白血球的穿越移動。關於白血球上是否會表現JAM-B的探討,目前仍很缺乏。本研究探討體外循環時是否會激發白血球表現JAM-B以及缺血-血液重新灌流是否會引起心肌血管內皮JAM-B的表現。方法:以流式細胞儀分析25位常規心臟手術經體外循環病人5個時間點的周邊血的血球JAM-B表現量,配合共軛焦顯微鏡觀察血球表面JAM-B,並以聚合酶鏈式反應檢測白血球 mRNA的變化。以共軛焦顯微鏡觀察3個時間點的心肌組織JAM-B表現,並以即時定量聚合酶鏈式反應檢測心肌組織 mRNA表現量的變化。結果:顆粒性白血球及單核淋巴球都有JAM-B的表現且隨體外循環時間的進行而有變化,尤其是在體外循環30分鐘及60分鐘時,JAM-B表現最強烈。心肌組織的血管內皮在缺血時及血液重新灌流後,JAM-B的表現量都有增加。結論:體外循環會刺激白血球JAM-B表現,不論是在mRNA或蛋白質的表現上。而缺血-血液重新灌流過程則會引起心肌組織血管內皮JAM-B的表現增加,以及心肌組織JAM-B mRNA的增加。
