Capsule Summary:•The rates of acute toxicity and infection during total skin electron beam therapy for mycosis fungoides are poorly delineated.•This large, modern series provides detailed toxicity information and demonstrates that the risk of cutaneous infection is higher than previously supposed.•Knowledge of these risks can help guide treatment of mycosis fungoides.How did this article change the practice of dermatology?This article showed that the well-known acute toxicities of total skin electron beam therapy include erythema/desquamation, blisters, hyperpigmentation, and skin pain. It also showed, however, that the risk of cutaneous infection during total skin electron beam therapy is significant as well, and patients should be educated to look for early signs of infection.1
