Abstract

Very little has been published about the intersection of living as both transgender and a plural system (more than one person sharing a body). This community-based participatory research study interviewed 15 participant systems. Using thematic analysis, a research team consisting of both systems and singlets (one person living in a body) developed five overarching themes at the intersection of transgender identity and plurality. These include embodiment, communication processes, affirming intersectional identities, living in a pluralphobic and transphobic society, and living authentically. Each system engaged in unique and effective internal communication strategies related to gender identities and presentations. Guidance and recommendations emerged for both those living as transgender and plural, as well as medical and mental health clinicians who seek to provide affirming care, which may reduce the marginalization experienced in the health care system and create more opportunities for equitable health outcomes.
