Abstract
A 61 year old man presented with acute right popliteal artery occlusion three times in 50 days due to an untreated, underlying dissection. Complete flow restoration and symptom relief were achieved three times by local thrombolysis. Initial ultrasonography, computed tomography angiography, and arteriography from different angles failed to unmask the underlying lesion. (A) Magnetic resonance angiogram after thrombolysis revealed an intimal flap (black arrow), suggesting an isolated spontaneous popliteal artery dissection with complete false lumen thrombosis. (B) Optical coherence tomography noted an intimal tear (white arrow). A covered self expandable stent (Viabahn; Gore, Flagstaff, AZ, USA) was deployed to cover the entire dissection.Image 1
