Abstract

Summary Introduction The non-obstructive azoospermia (NOA) is suspected with small testis and high serum FSH. According to the technique used, the rate of positive sperm retrieval was 36% to 72%. The decision of testicular biopsies with a high serum FSH on the day of oocyte pick-up is frequently discussed. Objective The purpose of this observation is to describe positive sperm retrieval with a high level of FSH on the day of oocyte pick with transfer of top quality embryos. Case report We report the case of a couple who came to infertility consultation at our IVF center in Aziza-Othmana hospital after two years of regular sexual intercourse with his partner. The men present a NOA confirmed by biological results and the woman has no problem of infertility. Conclusion The decision to make testicular biopsy with a high serum FSH in the same day of oocyte pick-up was not standardized. Sperm extraction in the same day of oocyte pick-up can be justified in patients with rare sperm. This procedure can optimize the transfer of top embryo quality and the pregnancy rate after ICSI.
