Abstract

This 4-year-old child presented with nonpalpable testicles noted since birth and an intact prepuce with megameatus. During bilateral inguinal exploration, both testicles were located at the internal ring. The right gubernaculum appeared thin and attenuated distally. A tissue band extended from the right testicle to the small intestine. Figure 1 demonstrates this intestinotesticular ligament. The ligament was transected and histology demonstrated dense connective tissue. Testicular biopsy results revealed absent germ cells in the left testicle and moderately reduced total germ cells (TGC) per tubule (T) (0.5 TGC/T) in the right testicle. Both testicles demonstrated a severely reduced Leydig cell count.
