Abstract

The National Service Framework for Older People (Department of Health (DoH), 2001) identifies intermediate care services as providing a necessary bridge between existing services in hospital and primary care. These new services will comprise information/advice to enable care in the home; intensive support in the home (hospital-at-home schemes); and short-term institutional care in a supportive environment such as a community hospital, nursing/residential home or sheltered housing. These services will be provided in conjunction with others such as diagnostic and ambulatory care, aids and equipment, home improvement support and carer support. An improved 24-hour provision of primary care together with better working relationships with accident and emergency departments is also planned to effect appropriate response at the time of need.
