Abstract

Summary Introduction: Retro-pharyngeal approach is useful in the management of upper cervical spine lesions. However the present of critical neurovascular structures explain the complexity of this approach.We made a report of patients with upper cervical spine lesions who have been operated using this approach. Observations:The first patient is a 46 years old housewife who presented a spastic tetraparesis and neuroimaging showed a C2-C3 cervical disc herniation with an intra-medullary hyper signal. The second patient is a 39 years old man received for hangman’s fracture after road traffic accident. The indication of anterior approach of the cervical spine was retained in both cases and realized in dual neuro-ORL team by a retro-pharyngeal approach. The outcome was satisfactory in both cases. Conclusion: Upper cervical lesions is rare and it is interesting to work in a double team because if anterior approach for the location of the lower cervical spine remains simple and easy for any neurosurgeon, it’s not the case for upper cervical locations.
