On 3 March 2016, the Secretary of State for Health, Jeremy Hunt, announced a major change to the patient safety infrastructure in the NHS with the setting up from 1 April 2016 of the independent Healthcare Safety Investigation Branch (HSIB). In a speech at the Global Patient Safety Summit (GPSS) on improving standards in health care in London, he also reflected on current patient safety initiatives (Department of Health (DH) and Hunt, 2016).
