Abstract

It is commonly accepted that a high proportion of research is wasted. Whilst well-designed randomised trials provide the most reliable evidence of treatment effect, narrow eligibility criteria and inflexible methodologies can limit generalisability. Pragmatic clinical trials aim to include a broader range of patients across diverse healthcare settings; this increases their relevance and thus impact. However, large trials take many years to design and complete.
