Abstract
Fatal haemorrhage following tracheostomy, though rare, is the most serious complication of a relatively simple procedure. It can cause massive aspiration leading to death. Various factors leading to innominate artery erosion by a tracheostomy tube are discussed along with a case report. An important cause of innominate artery erosion by a tracheostomy tube especially in children, is a very low tracheostome i.e. below the 4th tracheal ring.
