Abstract

In advanced stages of diffuse interstitial lung disease (DILD), the only treatment that may increase survival is lung transplantation. In Spain, 40% of all lung transplantations were performed in patients with DILD; it is the leading cause of transplantation in our country. The selection of candidates for lung transplantation continues to be a challenge for specialist physicians, especially due to ethical difficulties. In this protocol, we will analyze the criteria for referring patients with advanced interstitial lung disease to a lung transplantation center, the indications for their inclusion on the transplantation list, as well as the risk factors that we must take into account when evaluating this therapeutic measure and its contraindications in order to thus provide specific recommendations and optimize the management of these patients.
