Case presentation A 59-year-old woman with a history of palpitations and incessant narrow complex tachycardia was referred for electrophysiologic study and catheter ablation. Multipolar catheters were placed in the right atrium, His-bundle region, right ventricle, and coronary sinus (CS), with the proximal CS electrode positioned at the CS ostium.
