The study was conducted on the acute internal medicine service of a metropolitan general hospital. The basic instrument was a 35-item questionnaire answered daily by the patient's physician during the hospitalization. Specific information was elicited about physician care, nursing care, medications, consultation, laboratory and special diagnostic tests, X rays, and administrative circumstances during the previous 24 hours of hospitalization.
