Improving Standardized Hand-Off Communication Through Education for a Pediatric Neonatal Critical Care Transport Team

Abstract

Introduction: The Critical Care Transport Team at University of California Davis Medical Center Children's Hospital provides safe and expedient transport of critically ill children and neonates through collaboration with a Pediatric or Neonatal ICU attending physician (MCP). Clear and concise communication is essential in the care of these fragile patients. According to JCAHO, a standardized hand-off communication tool is necessary. It was determined that there was a need for a practice change and therefore an educational quality improvement project was initiated.
