Abstract

Advanced care planning is an important part of caring for elderly patients with multiple medical conditions, such as patients that participate in PACE (Program of All-Inclusive Care for the Elderly) programs. Traditional DNR (Do-Not Resuscitate) forms are limited to specifying the patients' desire to be resuscitated or not resuscitated in the event that they stop breathing and their heart stops beating. The MOLST (Medical Orders for Life-Sustaining Treatment) form allows patients to express their specific wishes regarding resuscitation, medical interventions, antibiotic use, and artificial feeding and hydration.
