Abstract

Objectives Communication between inpatient and outpatient providers is critically important as it provides opportunities for primary care providers (PCP's) and hospital medicine (HM) physicians to discuss information that may affect patient care. The lack of consistent, quality communication may contribute to errors, readmissions, and unnecessary testing.
