Abstract
When I had triple bypass surgery last fall, I left the hospital with a clear plan for what would happen next. I was enrolled in a cardio rehab program, and my primary care physician's care navigator called to remind me about screenings and appointments. I used my providers' patient portals to request appointments, send messages and track my progress.
