Abstract

Endoscopicdiverticulotomy using a double-beaked endoscope (Dohlman) has proved to be an invaluable aid in treating certain specific patients with Zenker's diverticulum (ie, patients who cannot undergo more than the shortest, least shocking, and least traumatizing procedures because of their cardiorespiratory, cerebrovascular problems, or other infirmities).1-3 One recognizes the benefits of the ingeniously developed double-beaked endoscope with the combined proximal and distal lighting. I know that it gives remarkably good visualization of the esophageal lumen, party wall, and diverticular sac. However, the difficulty encountered with the use of the double light yoke for proximal lighting, and the trouble obtaining light bulbs for the same, led me to modify the otherwise completely reliable instrumentarium by using fiber optic light carriers inserted into the proximal light carrier channels which resulted in even better lighting than experienced with the original double-yoke light carrier and bulbs.
