Abstract

There is an increasing demand for therapists to deliver psychotherapy via telehealth (i.e., through internet-based videoconferencing). Although this model of care offers many unique advantages, such as broadening access to empirically supported psychological treatments, it is crucial that therapists adhere to evidence-based principles of change while adapting to this novel delivery format. The Unified Protocol for Transdiagnostic Treatment of Emotional Disorders (UP) is an empirically supported, emotion-focused cognitive behavioral therapy (CBT) protocol that targets the temperamental vulnerabilities that underlie emotional disorders (e.g., anxiety disorders, unipolar mood disorders, etc.). The UP is organized into distinct modules that cover a range of cognitive and behavioral strategies that can be implemented effectively via telehealth. In this chapter, we describe basic logistical considerations for adapting the UP to an online delivery format. We then outline the principles of the UP, including its theoretical framework and specific guidance on how to adapt each module for telehealth delivery. Rationale for these adaptations is described so therapists can make informed decisions about how to adhere to this evidence-based protocol while meeting their patients’ individual needs. By addressing the strengths and challenges of adapting an evidence-based protocol to online delivery, we aim to better inform clinical practice both as it relates to the global COVID-19 pandemic as well as broader clinical contexts.
