The pilot sites were the irginia Hospital Center in Arlington nd the Veterans Affairs Medical enter in San Diego, CA. This article gives a background on he NCP and Model, the standardized anguage used in the nutrition diagnois step, medical record documentation, nd an explanation of how the two sites ame to participate in the pilot proram. It also provides a timeline for ach site’s implementation of the NCP, ncluding challenges faced and lessons earned.
