Abstract

Objective The purpose of this study was to determine whether upper anterior implants can provide torque control and bodily retraction of the anterior teeth in lingual orthodontic extraction cases. Methods 11 adult extraction cases treated with customized lingual appliance, were included in this study. 5 cases with normal inclination of upper central incisors (implant group) were used upper anterior implants for torque control. The implants were placed between the lateral incisors and the canines, and implants provide the vertical forces for the anterior teeth. The other 6 cases with labial inclination of upper anterior teeth treated without anterior implants were selected as the control group. Cephalometrics was used to analyze the torque changes of upper incisors before and after treatment, and compared with the control group. Results The result showed that the inclination of upper anterior teeth in implant group is (106.35±3.64)°.However, in the control group, the parameter is 2 standard deviation above the mean (112.38±4.48)°.The U1/NA in implant group (25.85±2.12)°, while in control group the U1/NA is (28.92±2.31)°.After space closure, upper anterior teeth were retracted obviously in both groups.U1/SN in implant group and control group decreased (10.82±3.14)° and (15.62±2.58)°, respectively. Moreover, U1/NA in implant group and control group decreased (10.85±3.59)° and (17.64±3.87)° , respectively. Compared with the control group, the retraction of upper anterior teeth in implant group is less. For vertical dimension, the anterior teeth in control group were slightly extruded, and U1-PP distance increased by 0.98±0.24 mm. But in implant group, there was not obvious extrusion of incisors, and the U1-PP distance only increased 0.39±0.22 mm. Conclusions In lingual orthodontics, anterior implant anchorage can control the torque of upper incisors and maintain the labial inclination of upper anterior incisors effectively in the process of teeth retraction.
