Abstract

Heart failure (HF) is associated with recurrent hospitalisations and high mortality. Guideline-directed medical therapy (GDMT) has been shown to improve prognosis in HF with reduced ejection fraction (HFrEF). Despite proven benefits of GDMT, there is still a substantial gap in its optimal use among Malaysian HF population. HF Medication Therapy Adherence Clinic (HF-MTAC) is a multidisciplinary programme involving trained pharmacists to provide care to HFrEF patients, which require resources for its implementation.
