Impact of fast-track surgery on perioperative care in patients undergoing hepatobiliary surgery.

Fast-track surgery (FTS) is a modern nursing approach that has gained popularity in the perioperative phase of surgery. To investigate the impact of FTS on perioperative care for hepatobiliary surgery. A retrospective analysis was performed on 98 patients who underwent hepatobiliary surgery and were admitted to our hospital from August 2021 to October 2023. They were divided into an observation group and a control group with 49 patients in each group according to different nursing directions. The control group was treated with standard nursing and the observation group was treated with FTS concept nursing. The length of hospital stay, visual analog scale (VAS) score, wound complications, nursing satisfaction, self-rating scale (SAS) score, and SF-36 quality of life (QoL) score were compared between the two groups before and after care. The duration of hospitalization, hospitalization cost, operation time, first implantation time, exhaust time, and first defecation time were shorter than the observation group (P < 0.05). Additionally, the observation group showed a significant difference between the VAS and SAS scores on days 1, 3, and 7 (P < 0.05). The complication rate in the observation group was 4.05% was significantly lower than the 18.36% in the control group, and the comparison groups were statistically significant (χ 2 = 5.018, P = 0.025). The observation group had a significantly higher level of nurse satisfaction (94.92%) than the control group (79.59%; χ 2 = 6.078, P = 0.014). Both groups showed higher QoL scores after nursing care, with higher scores in the observation group than in the control group (P = 0.032). FTS in patients undergoing hepatobiliary surgery can effectively improve negative mood, QoL, and nursing satisfaction; reduce wound complications; and accelerate patient rehabilitation.

Effects of chewing gum on gastrointestinal function in patients with intraspinal tumors after surgery

Objective To explore the effects of chewing gum on recovery of gastrointestinal function in patients with intraspinal tumors after surgery. Methods Totally 214 patients with intraspinal tumors treated in the Neurosurgical Ward of Spinal Cord and Spine of Beijing Tiantan Hospital, Capital Medical University from May 2018 to May 2019 were selected and divided into control (n=107) and observation (n=107) groups. Patients in the control group received conventional anti-constipation care, including postoperative dietary guidance, abdominal massage guidance and rehabilitation training, while patients in the observation group chewed 2 pieces of gum four times a day, each time for 10-15 min after awaking from anesthesia on the basis of conventional nursing care. Both groups received interventions for 7 d. The incidence of constipation and abdominal distension, time of first defecation and oral comfort scores after 7 d postoperatively. Results The incidence rate of constipation and abdominal distension was 7.48% and 15.89% in the observation group, lower than 25.23% and 37.38% in the control group, and there were statistically significant differences between the two groups (χ2=12.331, 12.650; P<0.05) . The time of first defecation was (19.22±5.64) h in the observation group, earlier than (27.08±6.61) in the control group, and there was statistically significant difference between the two groups (t=-9.366, P<0.05) . The oral comfort score after 7 d postoperatively was (8.59±1.13) in the observation group, higher than (5.12±2.66) in the control group, and there was statistically significant difference between the two groups (t=-12.420, P<0.05) . Conclusions Chewing gum can accelerate the recovery of gastrointestinal function in patients with intraspinal tumors after surgery and improve their oral comfort. Effects of nursing care in fast-track surgery on postoperative pain, psychological state, and patient satisfaction with nursing for glioma

BACKGROUNDThe brain is the most complex organ in the human body. Treatment for a glioma always involves a multi-disciplinary team. Nursing care in fast-track surgery or enhanced recovery after surgery is such kind of work implemented by an interdisciplinary team to provide services to patients to improve their outcomes. AIMTo explore the effects of nursing care in fast-track surgery on postoperative pain, psychological state, and patient satisfaction with nursing for glioma. METHODSFrom June 2018 to June 2020, 138 patients who underwent operation for glioma at Cancer Hospital Affiliated to Chongqing University were selected. They were categorized into groups according to different nursing care that they received. Of them, 69 patients receiving nursing care in fast-track surgery were included in an experimental group, and 69 patients receiving conventional postoperative nursing were included in a control group. Visual analogue scale was used to evaluate postoperative pain in the two groups immediately after the operation and at 3 d after the operation. Self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were used to evaluate the psychological status of patients immediately after operation and on the 3rd postoperative day. A self-made satisfaction scale for patient satisfaction with nursing was used to evaluate and compare patient satisfaction with nursing between the two groups. RESULTSTime to excretion, time to out-of-bed activities, and length of hospital stay were significantly shorter in the observation group than in the control group (P < 0.05). There was no significant difference in duration of operative time or intraoperative bleeding between the two groups (P > 0.05). There was no significant difference in postoperative pain score between the two groups (P > 0.05). The pain score was significantly lower in the observation group than in the control group at 3 d after the operation (P < 0.05). There was no significant difference in postoperative SAS or SDS score between the two groups (P > 0.05). SAS and SDS scores were significantly lower in the observation group than in the control group at 3 d after operation (P < 0.05). The rate of patient satisfaction with nursing was 94.2% in the observation group, which was significantly higher than that (81.2%) of the control group (P < 0.05). CONCLUSIONNursing care in fast-track surgery can relieve postoperative pain, anxiety, and depression, and improve patient satisfaction with nursing in patients with glioma, which is worthy of clinical application.

Effects of thumbtack-needle embedding therapy of auricular acupuncture on gastrointestinal function and intestinal microflora in patients with gastric cancer after operation

To observe the effects of thumbtack-needle embedding therapy of auricular acupuncture on gastrointestinal function and intestinal microflora in the patients with gastric cancer after operation, and to explore its mechanism. A total of 80 patients with gastric cancer after radical operation were randomly divided into an observation group (40 cases, 3 cases discontinued) and a control group (40 cases, 3 cases discontinued). The patients of both groups received the perioperative care for accelerating recovery. Additionally, in the observation group, the thumbtack-needle embedding therapy of auricular acupuncture was delivered at the auricular points of unilateral side, including Wei (CO4), Pi (CO13), Dachang (CO7), Xiaochang (CO6), Yuanzhong (AT2,3,4i), Erzhong (HX1), Sanjiao (CO17) and Jiaowozhong (TF3), and the needles were embedded and retained for 72 h. The postoperative recovery time of gastrointestinal function (the postoperative bowel sound recovery time, the first exhaust time, the first defecation time), the postoperative hospital stay and pain visual analogue scale (VAS) score were observed in the two groups. Before operation and on day 5 after operation, the serum gastrin level was detected in the two groups. The third-generation 16S rRNA sequencing technology was used to detect the composition and relative abundance of intestinal flora in the two groups before and after operation. Compared with the control group, the postoperative bowel sound recovery time, the first exhaust time and the first defecation time were shortened in the observation group (P<0.05). In the observation group, the VAS scores at 24 h, 48 h, and 72 h after surgery were lower than those of the control group, respectively (P<0.05). There was no significant differences in postoperative hospital stay and serum gastrin level between the two groups (P>0.05). The alpha diversity analysis showed that the differences in Shannon index, Simpson index, Pielou_J index and Pd_fath index were not significant statistically after intervention between the two groups (P>0.05). After intervention, the community structure of the fecal sample was similar at each taxonomic level between the two groups, and although the proportion between species was various, the difference was not significant (P>0.05). After intervention, there were 55 species with the differences between the two groups, 17 species of them presented significant difference in relative abundance in the observation group and 38 species in the control group. Regarding the level of genus, the levels of Klebsiell and Enterobacter increased (P<0.05) and the level of Streptococcus decreased (P<0.05) in the observation group. The main microbial groups that played an important role were Coprobacillaceae, Sutterellaceae and Yersiniaceae in the observation group. KEGG function prediction indicated that the function of intestinal microflora was mainly associated with the cofactor and vitamin metabolism, carbohydrate metabolism, and amino acid metabolism. The thumbtack-needle embedding therapy of auricular acupuncture improves the postoperative gastrointestinal function of the patients with gastric cancer probably through regulating the structure and relative abundance of intestinal microflora and affecting the energy metabolism.

Effect of Fushu plaster on the postoperative nursing in the patients with abdominal gastrointestinal surgery

Objective To evaluate the nursing value of Fushu plaster in the postoperative pain nursing of the patients with abdominal gastrointestinal surgery and the effect of promoting the gastrointestinal functional recovery.Methods One hundred and four patients with abdominal gastrointestinal surgery from January 2012 and October 2013 were chosen and divided into the control group and the observation group according to the random number table , each with 52 cases.The control group received the ordinary plaster , and the observation group received the Fushu plaster at the Zusanli and foot .The time of exhaust and defecation for the first time , and the postoperative pain evaluated by the visual analogue scale ( VAS) were compared between two groups . Results The scores of VAS 12 h, 24 h 48 h after the operation were respectively (3.65 ±1.00),(3.02 ± 0.62),(2.23 ±0.41) in the observation group, and were lower than those of the control group with (4.11 ± 0.79),(3.43 ±0.77),( 3.08 ±0.70), and the differences were statistically significant (t=2.59,2.99, 7.55, respectively;P〈0.05).The time of exhaust for the first time was (35.2 ±4.67) h in the observation group, and was (36.9 ±3.89 ) h in the control group , and the difference was statistically significant ( t =2.017,P〈0.05).The time of defecation for the first time was (40.7 ±5.32) h in the observation group, and was (41.4 ±3.89) h in the control group, and the difference was not statistically significant (t=0.765,P〉0.05 ) .Conclusions Application of Fushu plaster at the Zusanli and foot can improve the postoperative analgesia and promote gastrointestinal functional recovery , and is very convenient to use , and is worthy of clinical promotion . Effects of glutamine on intestinal functions of dogs with abdominal open injury coupled with seawater immersion

Objective To investigate the effects of glutamine on intestinal functions of dogs following abdominal open injury coupled with seawater immersion.Methods Following open abdominal injury,30 adult dogs were randomly divided into 3 groups:the seawater immersion group(the control group),the routine treatment group(the conventional group)and the glutamine treatment group(the observation group),each consisting of 10 animals.Blood pressure,heart rate,respiration,abdominal circumference,bowel sound were monitored both before and after injury.Blood samples were collected for the detection of endotoxin,D-lactate,diamine oxidase(DAO),tumor necrosis factor alpha(TNF-a),interleukin-1(IL-1 and lactulose/mannitol(L/M)ratio.After 24 hours,small intestine tissues were taken for pathological detection and measurement of intestinal mucosal thickness and villous height.Results When compared with the conventional group,no statistically significant differences could be seen in mean artery pressure(MAP),heart rate,respiration,bowel sound and abdominal circumference for the observation group(P>0.05).When compared with the conventional group and the observation group,significant differences could be noted in MAP and bowel sound for the control group(P<0.05).And statistical significance could also be noted for heart rate,respiration and abdominal circumference at hours 12 and 24 respectively following injury(P>0.05).Endotoxin:when compared with the conventional and observation groups,statistical significance could also be noted in endotoxin for the control group at hours 12 and 24 following injury(P>0.05),and statistical significance could also be noticed for the observation group at hour 24,when compared with the control and conventional groups(P>0.05).D-lactate:there were statistically significant differences for the conventional group,when comparisons were made between the 3 groups at hour 12 following injury(P<0.05),however,no statistical significance could be noted between the observation group and the control group(P>0.05).Statistical significance was observed for the conventional group,when compared with the control and observation groups(P<0.05).And no statistical significance was noted,when a comparison was made between the observation and control groups(P<0.05).DAO:when compared with the control and conventional groups,statistical significance could be noted for the observation group at hours 8 and 24 respectively following injury(P<0.05).TNF-α:when compared with observation and conventional groups,there was statistical significance for the observation group at hours 12 and 24 respectively following injury(P<0.05).IL-1:when compared with the control[(7.40±2.71)ng/L]and the conventional groups[(8.71±3.13)ng/L],statistical significance could be observed for the observation group at hour 12 following injury(P<0.05),however no statistical significance could be noted,when a comparison was made between the observation and control groups.At hour 24 following injury,when compared with the control[(9.91±3.18)ng/L]and conventional groups[(10.12±2.25)ng/L],statistical significance could be noticed for the observation group(P<0.05).Urine L/M ratio:statistical significance would be noticed for the observation group,when compared with the control and conventional groups(P<0.05).Pathological results:only mild edema could be seen in the intestinal mucosal cells of the animals in the observation group under optical microscope,which was obviously lighter,when compared with the observation and control groups.And there were statistical differences in intestinal mucosal thickness and villous height,when compared with the conventional group(P<0.05),however,no statistical differences could be noted,when a comparison was made between the control and conventional groups(P>0.05).Conclusions Glutamine seems to have definitive therapeutic effects on intestinal dysfunction induced by abdominal injury coupled with seawater immersion. Effects of multimodal analgesia guided by fast track surgery concept in knee arthroplasty

Objective To investigate the effect of multimodal analgesia guided by fast track surgery (FTS) concept in knee arthroplasty. Methods Sixty-five patients who underwent total knee arthroplasty were randomly divided into observation group and control group. The patients in the observation group underwent multimodal analgesia and pain management under the guidance of FTS, while the patients in the control group underwent perioperative management by traditional methods. The visual analogue scale (VAS) score, the amount of tramadol injection and the total days of hospital stay were compared between the two groups. Results The VAS score, the amount of tramadol injection, the total days of hospital stay of observation group were lower than those of control group, the differences were all significant (all P<0.05). Conclusions The application of multi-modal analgesia under FTS concept in perioperative management of total knee arthroplasty can effectively reduce the pain, the amount of pain medication and the total days hospital stay, which is worthy of clinical application. Postpartum rehabilitation massage and diet regulation in preventing postpartum deep venous thrombosis

Objective To explore the effect of postpartum rehabilitation massage and diet regulation in preventing postpartum deep venous thrombosis. Methods 104 cases of maternal treated in our hospital from January 2016 to June 2017 were selected as the research object, and were divided into observation group and control group with the random number table method. The control group (52 cases) received routine postpartum clinical intervention, while the observation group (52 cases) received postpartum rehabilitation massage and diet regulation. The postpartum incidence of lower extremity deep vein thrombosis, hemodynamic indexes, blood coagulation function, and other complications, uterine instauration condition, length of hospital stay, the time of exhaust and urination were compared between the two groups, the psychological condition was assessed with self-rating anxiety scale (SAS) and self rating depression scale (SDS), the sleep quality was evaluated with Pittsburgh sleep quality index scale (PSQI), the quality of life was evaluated with SF - 36 life quality scale, the maternal satisfaction on intervention was compared. Results The incidence of deep venous thrombosis was 1.92% in the observation group, which was lower than that in the control group (13.46%), and the total complication rate was 3.85%, lower than that in the control group (21.15%) (P<0.05). The postpartum hemodynamic indexes and coagulation function indexes of the observation group were superior to those of the control group (P<0.05). The excellent and good rate of uterine instauration condition was 100.00% in the observation group, higher than that in the control group (92.31%) (P<0.05). In the observation group, the time of hospitalization, exhaust, and urination were all shorter than those in the control group, SAS, SDS, PSQI, and SF-36 scores were all better than those in the control group (P<0.05); the satisfaction rate of the observation group was 96.15%, significantly higher than 82.69% of the control group (P<0.05). Conclusion Postpartum rehabilitation massage and diet regulation can significantly enhance the level of maternal postpartum care, and reduce the incidence of lower extremity deep vein thrombosis and the rate of other maternal complications, with high security, rapid recovery, good uterine recovery, good psychological state, improved sleep and life quality, and high maternal satisfaction rate, which can be applied. Effect of early stepwise nutrition management on feeding tolerance of postoperative patients with gastric cancer

BACKGROUNDPostoperative nutritional management of gastric cancer (GC) remains a problem that needs to be solved in clinical treatment.AIMTo develop an early graded nutrition management plan and evaluate its impact on feeding tolerance, nutritional status, and prognosis.METHODSIn total, 142 patients who underwent laparoscopic radical gastrectomy at Jiujiang University Affiliated Hospital between August 2021 and August 2022 were included in this study. Based on postoperative nutritional management and feeding, the patients were divided into observation and control groups. The general information questionnaire, Visual Analog Scale, and Pittsburgh Sleep Quality Index were used to evaluate pain and sleep of patients, respectively. Independent sample t-test and χ2 test were used to analyze differences between groups.RESULTSThe feeding intolerance rates in the control and observation groups were 13.2% and 4.1%, respectively. Hospitalization time and first defecation times in the observation group were shorter than those in the control group. Hemoglobin, prealbumin, transferrin, and immunological indices in the observation group were significantly higher than those in the control group 7 days after surgery, whereas calcitonin levels were significantly lower than those in the control group (P < 0.05). In general, the nutritional status of the observation group was better than that of the control group, and pain and sleep quality scores improved.CONCLUSIONCompared with the conventional postoperative feeding, early stepwise nutritional management can significantly enhance the nutritional status of patients with GC after surgery, improve their feeding tolerance, and reduce postoperative complications.

Comparison of Iso-C 3D navigation system versus X-ray fluoroscopy in minimally invasive surgery for Kümmell disease

Objective To compare the Iso-C 3D navigation system versus the traditional X-ray fluoroscopy in minimally invasive surgery for Kummell disease. Methods Included for the present retrospective study were 42 patients with Kummell disease who had been treated by percutaneous kyphoplasty (PKP) or percutaneous vertebroplasty (PVP) at Department of Spine Surgery, Honghui Hospital from October 2016 to June 2018. They were 11 men and 31 women, aged from 43 to 72 years (average, 57.4 years). The operation was aided by Iso-C 3D navigational system in 21 patients (observation group) and by X-ray fluoroscopy in the other 21 patients (control group). The 2 groups were compared before operation, one day and 6 months after operation in terms of visual analogue scale (VAS), Oswestry disability index (ODI), cobb angle and anterior vertebral height. Results The 2 groups were comparable because there were no significant differences between them in the preoperative general data (P>0.05). All the patients were followed up for an average of 9.6 months (from 6.0 to 18.5 months). At one day after operation for the observation and control groups, the VAS scores (2.2±0.7 and 3.9±1.5), ODI scores (15.6±2.8 and 26.2±6.9), cob angles (19.5°±1.7° and 20.8°±0.8°) and anterior vertebral heights (20.7±1.4 and 18.7±1.5 mm) were all significantly improved compared with those before operation (P 0.05). The VAS, ODI, cobb angle and anterior vertebral height at one day after operation for the observation group were all significantly better than those for the control group (P 0.05). No complications were observed in the observation group but 6 cases in control group reported cement leakage. Conclusion In the PKP/PVP treatment of Kummell disease, Iso-C 3D navigational system may be superior to traditional X-ray fluoroscopy in that it can lead to better recovery of anterior vertebral height, kyphosis correction, pain relief and quality of life. The Impact of Operating Room Detailed Nursing Based on Risk Management on Surgical Patient Satisfaction and Intraoperative Risk Control

Background The operating room has always been a key clinical inspection high-risk department. The “;Detailed Rules for the Implementation of Evaluation Standards for Tertiary General Hospitals” issued by the Ministry of Health in 2011 emphasized the establishment of operating room quality and safety indicators, which can be evaluated regularly, and continuous improvement is required. The nursing level of nurses in the room is directly related to the overall nursing quality of the hospital. Objective To observe the impact of detailed nursing in operating room based on risk management on surgical patient satisfaction and intraoperative risk control. Methods A retrospective selection of 130 patients who were treated in the operating room of our hospital from January 2018 to March 2020 was retrospectively selected. Among them, 65 patients used conventional operating room nursing procedures, and the other 65 patients used risk management-based operating room detailed nursing procedures. The intraoperative rescue, secondary intubation, incidence of &gt; 3 h in room, nursing error rate, postoperative general situation (recovery time of bowel sounds, first exhaust time, ambulation time, postoperative complication rate) and satisfaction were compared between the two groups. Visual analogue scale (VAS) score was used to evaluate the degree of postoperative pain. Nursing quality score and health survey short form (SF-36) were used to evaluate nursing quality and quality of life. Results: Intraoperative rescue (0.00%), secondary intubation (0.00%), in-room&gt; 3h incidence (1.54%) and nursing error rate (0.00%) in the observation group were compared with those in the control group, which were not statistically significant (P &gt;0.05). The recovery time of bowel sounds in the observation group was (41.71 ±3.46) h, the time to first exhaust (59.47±5.23) h, and the time to get out of bed (54.36±4.78) d were shorter than those in the control group. The postoperative complication rate (3.08%) was lower than that of the control group, which had statistical significance (P&lt;0.05). The VAS scores of the observation group at 6h, 12h, and 24h after surgery were lower than those of the control group, and the satisfaction level of the observation group (93.85%) was higher than that of the control group, which had statistical significance (P&lt;0.05). Observation group’s nursing quality score (nursing skills, environmental management, nursing quality monitoring, disinfection and isolation, nursing document management), quality of life score (physical function, social support, pain, mental health, social function, mood, mental state, general health) All were higher than the control group, which had statistical significance (P&lt;0.05). Conclusion: The application of operation room detail nursing based on risk management in surgery can reduce postoperative pain, promote the recovery of gastrointestinal function, improve patient satisfaction and quality of life, and effectively control intraoperative risks.

To observe the curative efficacy of auricular comprehensive therapy on menstrual migraine(MM) and its effect on serum prostaglandin F2α(PGF2α), prostaglandin E2(PGE2) contents and ratio, so as to explore its possible mechanism. A total of 66 patients with MM of liver-fire syndrome were randomly divided into observation group (33 cases, 2 cases dropped off) and control group (33 cases, 2 cases dropped off), and 20 healthy women were included in the normal group. Patients in the control group were given flunarizine hydrochloride capsules orally, twice a day, for 3 consecutive weeks. Patients in the observation group were treated with auricular comprehensive therapy, starting 2-5 days before menstrual cramps, once a week, for a total of 3 weeks. The visual analogue scale (VAS) and migraine score were evaluated before and after treatment, and follow-up for 1 and 2 menstrual cycles. Serum PGF2α and PGE2 contents were measured before and after treatment, and the PGF2α/PGE2 ratio was calculated. The clinical effective rates in the two groups were calculated. After treatment and follow-up for 1 and 2 menstrual cycles, the VAS scores, headache degree, the frequency and duration of headache attacks, as well as accompanying symptoms of the observation and control groups were lower than those before treatment(P<0.05), and those of the observation group was lower than those of the control group(P<0.05). Before treatment, the PGF2α contents in the observation and control group were significantly higher(P<0.05), while the PGE2 contents lower(P<0.05) and PGF2α/PGE2 ratio higher(P<0.05) than those in the normal group. After treatment, the serum PGF2α contents in the observation and control group were significantly reduced compared with which before treatment(P<0.05), and were lower in the observation group than that in the control group (P<0.05). The serum PGE2 contents in the observation and control groups were significantly increased after treatment compared with which before treatment(P<0.05), with the contents in the observation group higher than that in the control group(P<0.05). The serum PGF2α/PGE2 ratio in the observation and control group was significantly reduced after treatment compared with which before treatment(P<0.05), with the control group higher than the normal group(P<0.05), and the observation group lower than the control group(P<0.05). The clinical effective rate of the observation group was 93.5% (29/31), and that of the control group was 77.4% (24/31). The effective rate of the observation group was significantly higher than that of the control group(P<0.05). The curative efficacy of auricular comprehensive therapy on MM with liver-fire syndrome is significantly better than that of oral flunarizine hydrochloride capsules, especially in relieving hea-daches, reducing the frequency and duration of headache attacks, as well as accompanying symptoms. Its mechanism may be related to regulating the abnormal PGF2α and PGE2 contents of patients and reducing the ratio of PGF2α/PGE2.

Effects of fast track surgery nursing on fatigue and quality of life among hepatocellular carcinoma patients

Objective To explore the effects of fast track surgery (FTS) nursing on fatigue and quality of life among hepatocellular carcinoma patients. Methods From June 2017 to June 2018, we selected 80 primary hepatic carcinoma patients with hepatocellular carcinoma resection of hepatobiliary surgery at Peking Union Medical College Cancer Hospital as subjects by convenience sampling. All of the patients were divided into control group and observation group with the method of random number table, 40 cases in each group. Observation group carried out FTS, while control group adopted routine nursing. The intervention effects were compared with the Cancer Fatigue Scale (CFS) and the Quality of Life Questionnaire Core 30 (QLQ-C30) . Results Before intervention, there was no statistical difference in the score of CFS between two groups (P>0.05) . After intervention, the total score and dimension scores of CRF of observation group were lower than those of control group with statistical differences (P 0.05) . After intervention, the total score of QLQ-C30 of observation group was higher than that of control group with a statistical difference (P<0.01) . Conclusions FTS can effectively improve the postoperative fatigue of hepatocellular carcinoma patients and quality of life of patients which is worthy of clinical popularization and application. Effect of Endoscopic Retrograde Cholangiopancreatography Lithotomy Combined with Laparoscopic Cholecystectomy on Pain and Prognosis of Patients with Gallstones and Extrahepatic Bile Duct Stones

Objective The researchers aim to assess how endoscopic retrograde cholangiopancreatography lithotomy and laparoscopic cholecystectomy affected pain and prognosis in individuals with gallstones and extrahepatic bile duct stones. Methods Researchers studied 100 persons with gallstones and extrahepatic bile duct stones from January 2016 to August 2021. They were split into two groups: control and observation. The control group underwent open cholecystectomy + choledocholithotomy + T-tube drainage. There were ERCP + lithotomy + Laparoscopic cholecystectomy procedures conducted on the observation group of patients (LC). The operation time, incision size, and bleeding volume; stone clearance rate and stone recurrence rate; visual analogue scale (VAS); Eating time, exhaust time, out of bed activity time, and postoperative hospital stay; the two groups' difficulties were contrasted and studied. Results The observation group's operation duration was much longer than the control group's, and the incision size and bleeding volume were significantly reduced in the observation group. When it came to recurrence, there was a significant difference between the observation group (2.0%) and the control group (4.0%), even though stone clearance did not differ substantially (20.00%). The VAS scores 3 and 7 days after surgery were significantly different between the observation and control groups. The observation group's eating time, exhaustion, activity time, and postoperative hospital stay were much shorter than the control group's; complications occurred at a lower rate (20%) in the observation group than in the control group (34%). Conclusion When gallstones and extrahepatic bile-duct stones are removed using laparoscopic lithotomy and laparoscopic cystoscopy (LC), the operation indices are improved and the risk of recurrence of stones is reduced and reducing the occurrence of complications, all of which improve patient outcomes and deserve to be considered a clinical reference.

Fast track surgery nursing models accelerate physical rehabilitation in perioperative patients undergoing hepatobiliary surgery.

To compare the effects between primary hepatic carcinoma (PHC) patients undergoing laparoscopic partial hepatectomy receiving Fast track surgery (FTS) nursing and traditional nursing, aiming to select an effective nursing intervention plan for such patients. This study included 84 patients with PHC who underwent laparoscopic partial hepatectomy, randomly divided into an observation group (n42) and a control group (n=42). The observation group received FTS nursing, while the control group received traditional nursing care. Intraoperative and postoperative conditions, serological indicators, and complication rates were compared between the groups. The observation group showed significantly reduced postoperative drainage tube removal time, anal exhaust time, hospitalization expenses, and length of stay (LOS) compared to the control group (all P<0.05). C-reactive protein (CRP), total bilirubin (Tbil), alanine aminotransferase (ALT), and cholinesterase (ChE) levels in the observation group were notably lower than those in the control group on postoperative days 1, 3, and 5 (all P<0.05). No significant difference was found in postoperative complication rates between the observation (14.29%) and control groups (16.67%) (P>0.05). Compared with traditional nursing, FTS nursing effectively reduces inflammation and liver function injury in patients undergoing PHC surgery, shortens LOS, lowers hospitalization expenses, and accelerates physical rehabilitation without increasing postoperative complications.
