Abstract

Despite the availability of specific antidigoxin Fab fragments that represent the best antidote in case of severe poisoning, morbidity and mortality from digitalis poisonings remain still important. Antidigoxin Fab should be administered as first-line therapy, if atropine is unsuccessful. The mechanism of action of this antidote is based on digitalis immunoneutralisation and redistribution from their target tissue to the vascular compartment. To date, no study has shown any benefit to reduce the mortality rate of poisoned patients. However, it is not doubtful that an analysis of prognostic factors in digitalis poisoning may help for a better indication and prescription of antidigoxin Fab fragments. Several questions are still discussed, such as the optimal quantity of Fab fragments to administer and the best modality of their intravenous infusion.
