Abstract

Long-standing ischemic priapism leads to cavernosal smooth muscle necrosis and erectile dysfunction [1]. We report a 28-year-old gentleman who presented with 3-week duration of neglected idiopathic ischemic priapism. As his stretched penile length was 9 cm, he was taken up for immediate implantation of penile prosthesis. With minimal difficulty in corporal dilation due to scarring, a 3-piece inflatable penile prosthesis was placed. He developed fever with chills and generalized rash in the immediate postoperative period and it responded to change of antibiotics and antihistaminics. He has good intercourse satisfaction with antegrade ejaculation at the end of 2 years.
