Abstract

A 30-year-old male injection drug user was admitted with a swollen, painful left buttock and thigh after injection of buprenorphine. On examination, we found a tennis-ball-sized mass in his left medial upper thigh, with erythematous, tender skin (Figure 1), and decreased flexion and external rotation of his left hip. The patient was febrile and sweating profusely. Conspicuous laboratory results were a C-reactive protein of 279 mg/L, a white cell count of 21.2×109/L, and platelet counts of 626×109/L.
