Figure 1. Diaper Diagnosis of Porphyria. A blood- and urine-soaked diaper from a neonate with organomegaly, hemolytic anemia, thrombocytopenia, and cutaneous blisters was photographed under ambient light with a flash apparatus (Panel A) and under Wood's light (320 to 420 nm) (Panel B). The child's parents were first cousins, but the family history was otherwise negative for porphyria. Subsequent investigation showed very high levels of porphyrins in urine, serum, red cells, and stool. Fluorescence of a diaper under Wood's light is an indication that further studies of porphyrin metabolism should be done.
