The involved portion of the kidney enlarges and if not operated upon, may penetrate into the perinephric space, or, as in our case, between the peritoneum, where it gives rise to localised adhesive peritonitis and abdominal perivisceritis. The symptoms are of gradual onset with fever, vague malaise, and developing lumbar or abdominal pain. There is often a history of boils, carbuncles, or paronychia, or sometimes only of small cutaneous suppurations, two to eight weeks prior to the symptoms.
