•. Hysterosalpingography (HSG) is the first-line imaging tool in the investigation of infertility.•. It remains the investigation method of choice in the assessment of tubal patency despite advances in ultrasound, MRI and CT imaging.•. Uterine and peritubal abnormalities can also be detected on HSG.•. Other indications for HSG include post-operative evaluation of tubal occlusion following tubal ligation and (more recently) Essure sterilisation, and evaluation following reversal of tubal ligation.Hysterosalpingography (HSG) is a first-line investigation for infertility, and is used predominantly to assess tubal patency as tubal occlusion remains one of the most common causes of female infertility.
