Spontaneous intracranial hypotension is an infrequent cause of secondary headache due to a CSF hypovolemia, that occurs in the absence of known dural tear. Associated simple clinical presentation, orthostatic headache, and characteristic MRI findings have resulted these last years in an improved recognition of this syndrome. The spectrum of clinical presentation has increasingly broadened, with numerous ambiguous atypical presentations. The management - based on epidural blood patch - is getting better codified.
