HP-09-002 Grafting procedure for Peyronie's disease: buccal mucosa vs autovein

Abstract

Surgery remains the gold standard treatment for men with stable Peyronie's disease (PD). The aim of this study was to compare the use of autovein and buccal mucosa for grafting procedure in the treatment of acquired penile curvatures. We retrospectively evaluated 123 patients surgically treated at our institution for PD with corporoplasty, plaque incision and grafting procedure with buccal mucosa and autovein between 2008 and 2015. 82 patients underwent grafting procedure with saphenous allograft (group A) and 41 patients – with buccal mucosa graft (Group B). There were no significant differences between two groups prior to the surgery. There was significant difference in mean followup for group A vs for group B, 70 months and 46 months, respectively. It is caused with rather recent use of buccal mucosa for corporoplasty. Total operative time for harvesting and application of the graft was more in saphenous graft than for buccal mucosa graft (p≤0.05). Complete penile straightening in group A was achieved in 56% patient’s vs 63% in group B (p≥0.05). There were no donor-site complications in both groups, cosmetically donor site was better in buccal mucosa graft site. Overall satisfaction rate due to our questionnaire was 83% in group A and 85% - in group B. There were no significant differences between groups in complication rates, such as decrease in glans sensitivity, penile shortening and recurrent deformity.
