Abstract

to evaluate the management of patients (pts) and post-operative outcome after sex related urethral rupture. Penile fracture is an extremely uncommon urologic injury with approximately 1331 reported cases in the literature till the years 2001. To best of our knowledge from 2001 up today, 1839 more cases have been reported, only in 159 of them anterior urethral rupture was associated. We present 13 new cases.we evaluated all the pts underwent surgery after coital penile fracture in 5 italian centers. Were evalued with after surgery urethrography, uroflowmetry 60 days and with validated test to evaluate erectile function and urinary flow and dynamic penile Doppler ultrasound (US) 6 month after surgery. In all the 13 pts evaluated (age 26-61 years) physical examination revealed subcutaneous hematoma associated with pain, and urethral bleeding (70%) or voiding symptoms (30%). Penile fracture was confirmed (55% monolateral, 45% bilateral) with US whereas urethral rupture was revealed with urethrography. Emergency surgery was performed (meanly 7 hours after trauma) through sub coronal approach in 8 pts, peno-scrotal in 4 pts or on the penile raphe in 1 case. Urethral trauma was repaired with direct suture in case of partial disruption (65%), or an end to end anastomosis in case of complete disruption (35%). In all cases an albugineal suture was performed. At follow up the catheter was manteined mainly for 24 days(12-50). Uroflowmetry showed a mean maximum flow of 20 ml/sec (15-25), the questionnaires IPSS e IIEF-5 reached a mean score of 4/35 (2-9) and 22/25 (20-25) respectively. Urethrography demonstrated in 2 cases a slight substenosis. Dynamic penile Doppler US was normal in all cases.
