This guide summarises the keystone perforator island flap (KPIF) in lower limb reconstruction from the groin to the ankle.Depending on the anatomical site and the dermatome locations, the KPIF can be rhomboidal or rectangular in shape. Welcome to the first of a series of articles in which AJOPS invites plastic surgeons from Australia and New Zealand to share their surgical expertise and methodologies for performing complicated plastic surgical procedures. Since its introduction in the early 1990’s, Associate Professor Felix Behan’s keystone perforator island flap (KPIF) has proved itself to be a valuable addition to the armamentarium of plastic surgeons in closing difficult reconstructive defects. The KPIF is extremely useful in the closure of defects of the lower limb.
