Abstract

Forty patients with preoperative systolic pulmonary pressures exceeding 80 mm Hg were analyzed by recatheterization four months to five years postoperatively. Thirty-two were found to be asymptomatic. Improvement in heart size and pulmonary vascular markings were noted, as well as reduction of pulmonary hypertension. In 39 patients reduction of pulmonary flow to normal value occurred. Total pulmonary resistance fell 30%. In five of six patients undergoing repeated postoperative catheterization over an eight-year period, progressive improvement of hemodynamic measurements was noted. Of the 40 patients having transatrial closure of highpressure defects, 39 (91%) survived. The value of an aggressive surgical approach to high-pressure high-resistance ventricular septal defects is emphasized.
