Abstract

In 1998 the national HIV prevalence amongst women attending antenatal clinics in South Africa increased from 16% to 23% and HIV rates amongst sex workers in urban areas were reported at close to 50%. In the last decade, sex work in central Johannesburg has increased significantly. Despite the growing industry and high rates of HIV and STDs, there is little information on the sex work trade in Johannesburg. Further, there are limited or no targeted health services for sex workers in this area. Focus group discussions were therefore conducted amongst four groups of female sex workers in inner-city Johannesburg at an STD/family planning clinic. Participants were asked to discuss how they entered the industry, their condom usage, exploitation/abuse, HIV/AIDS infection, and health and social services needed. Major themes of the focus groups were abstracted from the translated and transcribed texts. The major problems identified by this study were: clients' reluctance to use condoms, police abuse, lack of sex worker friendly health services, and HIV positive sex workers. The outcomes of these discussions will be used to help inform future health policy and interventions amongst sex workers in this area. En 1998, la prevalence nationale du VIH chez les femmes qui frequentaient les cliniques antenatales en Afrique du Sud a augmente de 16% a 23% et les taux du VIH parmi les prostituees dans les agglomeration urbaines ont augmente jusqu'a 50%. Au cours de la derniere decennie, la prostitution a Johannesbourg Central a augmente de maniere significative. Malgre le fait que l'industrie grandit et malgre les taux eleves du VIH et des MST, il y a peu de renseignement concernant la prostitution comme metier a Johannesbourg. En plus, il n'existe pas ou bien il y a tres peu de services de sante qui visent les prostituees dans cette region. Les discussions en groupes cibles ont ete menes donc parmi quatre groupes de prostituees dans une clinique MST/Planning familial situee au centre de Johannesbourg. Les participants ont ete demandes de discuter la maniere dont elles sont entrees dans l'industrie, leur emploi de preservatifs l'exploitation/l'abus, l'infection du VIH/SIDA, et les services sociaux et de sante dont elles avaient besoin. Les themes principaux des groupes cibles ont ete tires des textes traduits et transcrits. Les problemes importants identifies par cette etude etaent: la repugnance de la part des clientes a utiliser des preservatifs, l'abus par la police, le manque de services de sante qui favorisent la prostitution, et le probleme des prostituees atteintes par le VIH. Les resultats de ces discussions serviront a promouvoir la politique et les interventions futures de la sante parmi les prostituees dans cette region.
