Abstract

Hepatitis C is on the rise nationwide despite the goal to eliminate it. Only 17% of patients are linked to care with the standard cascade of care (SOC); therefore, new models of care are needed. This research evaluated a model (HepCare) for linkage and engagement in hepatitis C care using the regular care team. A telemedicine visit was conducted to link people to care. Completing a hepatitis C evaluation demonstrated engagement. Linkage in care rates for the HepCare group were 100% compared with the 63% in the SOC group. Primary Care is the ideal setting to initiate the HepCare model.
