Abstract

Objective To study the clinical value of hepatic artery resection and reconstruction in hilar cholangiocarcinoma.Methods Clinical data of 23 hilar cholangiocarcinoma cases surgically treated in Suqian People's Hospital from January 2007 to December 2012 were reviewed retrospectively.In all these cases,the portal vein was resected and reconstructed.Hepatic artery communis and/or hepatic artery propria was resected and reconstructed in 16 cases,not reconstructed in 7 cases.Results In the 16 cases in whom the hepatic artery and/or proper hepatic artery was reconstructed,biliary fistula occurred in 2 cases,biliary infection occurred in 1 case and incision infection occurred in 2 cases,there was no perioperative mortality,while in the 7 cases not undergoing artery reconstruction,there were 3 cases of postoperative biliary fistula,1 case of biliary tract infection,3 cases of incision infection,and 2 cases died during perioperative period.Conclusions Hepatic artery resection and reconstruction can increase the cure rate of hilar cholangiocarcinoma and decrease perioperative morbidity and mortality.Vascular invasion is not the contraindication to radical resection of hilar cholangiocarcinoma.
